Each Credit Union member age 18 years or older is eligible for $2,000 accidental death and dismemberment insurance coverage at no cost to the member. However, the member must enroll in the program by completing the accident insurance enrollment form and by sending it in to the insurance company. A certificate of insurance will be mailed to the member as evidence of coverage. Coverage is NOT automatic and the member must have the insurance certificate as proof of coverage.

Higher dollar amounts are available, but only if the member requests and pays for that additional coverage. Standard coverages are:
        $2,000 for loss of life
        $2,000 for loss of two members (hand, foot or eye)
        $2,000 for loss of speech and hearing
        $2,000 for loss of entire sight in both eyes
        $1,000 for loss of entire sight in one eye
        $1,000 for loss of one hand or foot
        $1,000 for loss of speech or hearing
        $500 for loss of thumb and index finger on same hand

Exclusions: This insurance is in force as long as you are a member of the Credit Union and the Master Policy is in force. The loss of a hand or foot means complete severance through or above the wrist or ankle joint; for thumb or index finger, it means complete severance through or above the metacarpophalangeal joints. The loss of sight of eye means irrecoverable loss of entire sight. The loss of speech means complete and irrecoverable loss thereof; for hearing it means complete and irrecoverable loss of hearing in both ears.
